Healthy Futures Focus Area: Instrument Packets for Pilot Year 1

The Healthy Futures Focus Area refers to unmet health needs within communities including access to health care, preventive and primary health care, health literacy, and reducing childhood obesity.

There are three packets in the Healthy Futures Focus Area. Each packet consists of one or more national performance measures and includes:

  • Description of the national performance measures covered by the packet
  • Definitions of key terms
  • Considerations for measuring the national performance measures
  • Data collection challenges and strategies
  • Data collection instruments
  • Instructions for using the instruments

All of the packets address national output measures. Remember, you are required to identify and measure an intermediate outcome that aligns with the output you select and that supports your service activity.

Healthy Futures Outputs H1, H2, and H3 (PDF)
H1. Number of individuals who are uninsured, economically disadvantaged, medically underserved, or living in rural areas utilizing preventive and primary health care services and programs. 
H2. Number of clients to whom information on health insurance, health care access, and health benefits programs is delivered. 
H3. Number of clients enrolled in health insurance, health services, and health benefits programs.

Healthy Futures Outputs H4, H5, and H6 (PDF)
H4. Number of clients participating in health education programs. 
H5. Number of children and youth engaged in in-school or after school physical education activities with the purpose of reducing childhood obesity. 
H6. Number of children and youth receiving nutrition education with the purpose of reducing childhood obesity.

Healthy Futures Output H7 (PDF)
H7. Number of clients receiving language translation services at clinics and in emergency rooms.
